Export Preparation Questionnaire

  • Traveling with a pet outside of the United States is becoming increasingly common. However, export requirements vary greatly from country to country and the documents and requirements that need to be met vary as well. Some of the factors are the species, number of pets, purpose of travel, and method of transportation. In order to best serve you and your pet, we will research current requirements for you to make your trip as easy as possible. IF YOUR PET ARRIVES IN A COUNTRY WITHOUT MEETING ENTRY AND EXPORT REQUIREMENTS, YOUR PET CAN BE CONFISCATED FROM YOU, PLACED IN QUARANTINE, AND/OR SENT BACK TO THE UNITED STATES AT YOUR EXPENSE.

  • For pet export, most travel needs to be planned several months in advance. Please be prepared to bring your pet in several times, as many countries require multiple visits for different tests (such as testing for contagious disease and/or sufficient vaccine protection), final health examinations, vaccine administration, and deworming. Some tests required take several months to process and receive results.

  • Please fill in the following information as completely and accurately as possible. If your travel arrangements have not yet been finalized or you are uncertain, please indicate this for the applicable question.

Is this a permanent move or will your pet be returning to the United States?
Will your pet be traveling on the same plane as you?
If your pet will NOT be traveling with you:
Will your flight make ANY stops between Ohio and your destination country or will you be driving to an airport outside of Ohio to fly?
Has your pet received vaccinations at another clinic? 

In many instances proof of multiple vaccines is required, the most common being the rabies vaccine.


If you have your pets' rabies vaccine certificates or other vaccine records, please bring them to your appointment. 


Originals will be required for export.
